Pertinent parts of your medical record will be available to you, your family if you wish, and all of your medical physicians 24 hours a day 7 days per week via a special personal health record (PHR) created especially for you. Your medical record will be both private and secure and you will have complete control over who can access this information. In addition, your ePACE consultant will be directly available to you 24/7 through telephone or electronic communication to answer questions and offer guidance regarding any aspect of your Personal Health Record.
